Welcome to the Health Office!

My name is Carly McGannon. I am a Registered Nurse for the Town of Trumbull Nursing Department. This is my second year assigned to St. Theresa’s and it is a pleasure to be able to serve the St. Theresa School community.

As the School Nurse, I work within a framework mandated by the State of Connecticut for both public and private schools. My standing orders are dictated by the Trumbull Medical Director and are authorized by the town’s Medical Advisory Board.

Absences: If your student is absent due to illness, please notify the nurse in the health office via phone or email.

Illness: Please keep your student home if they are sick!

• For fever (>100) your student can return to school after being fever-free for 24 hours without the use of fever-reducing medications.

• For vomiting, your child must stay home for 24 hours and until symptoms resolve.

Injuries/Surgeries:

- Please communicate any significant injuries that occur at home (bruises, burns, or any facial injuries).

- Any visits to urgent care or the ER, scheduled surgery, and all devices (casts, walking boots, splints, stitches, staples, glue) require a doctor’s note to return to school, noting any activity restrictions (specifically whether they are allowed to participate in physical education or recess/playground).

Medications: All medications, including commonly used over the counter medications and creams, require a ‘Medication Authorization’ form completed by the doctor and signed by the parent.

- Medications may not be sent in backpacks; they must be hand delivered by an adult.

- Over the counter medication must be in the original, unopened container.

Prescribed medication must have a current prescription labeled with the child's name, medication name, physician’s name, and administration directions clearly marked on the container.

Physicals/Immunizations:

- PreK: Physical Exams are required upon enrollment and annually. A physical examination and up-to-date immunization record is required prior to entry into the Preschool Program.

- Kindergarten: A physical examination and up-to-date immunization record is required prior to entry into kindergarten. In addition, any child who is less than 5 years old at the time of kindergarten entrance is also required to submit a physical exam within 30 days of their 5th birthday.

K-12 Health Assessment Form

- Grades 3 and 6:

A physical examination and up-to-date immunization record is required
between 6/1/2022 and 6/1/2023.

*Students will be excluded from school if the requirement is not met by 6/1/2023. Students may return to school once the requirement is met.

- Grades 7:

Tdap and Meningococcal immunizations are required before starting 7th
grade; typically given at the 6th grade physical appointment.

2022-2023 Immunization Requirements

- Transfers In: A physical examination and up-to-date immunization record is required to start school, regardless of grade.
- Transfers Out: A release of records form must be completed for medical records to be sent to the new school.

Flu Shots/Exemptions:
- Pre-K Requirements: The State of Connecticut requires the influenza vaccine for all preschoolers, 1 dose administered each year between August 1st and December 31st (2 doses separated by at least 28 days required for those receiving flu for the first time).

- Medical Exemptions: Medical exemptions will only be accepted with appropriate documentation. They must be on ‘Connecticut Department of Public Health Medical Exemption Certification Statement’ form along with documentation from the physician.

School Nurses
School nurses are licensed nurses who work in public and private schools and use evidence-based practice to promote individual and population-based student health, provide care coordination, advocate for quality student-centered care, and advance academic success. School nurses are leaders who bridge health care and education, and collaborate to help create healthy
communities. (NASN, 2017). School Nurses use clinical knowledge and judgment to oversee school health services, policies, and programs and promote health education.

Responsibilities
● Assess the ill and injured on the school campus and administer First Aid as needed
● Prevent, control and track communicable diseases
● Enforce State and Board of Education policy for compliance of immunizations and
mandated school physicals
● Provide health screenings based on grade level (vision, hearing and scoliosis)
● Maintain student health records, prepare reports & surveys as directed by the School
Nurse Coordinator and Director
● Coordinate chronic health condition management and collaborate with school staff
● Connect school community with health resources
● Administer medications authorized by physicians and parents
● Advocate for the health rights of children and their families

● Develop and maintain Individualized/Emergency Health Care Plans for students with chronic & acute health conditions; participate in 504 plans and PPTs
